This is a fantastic newly created opportunity, working for one of the leading mental health charities in Hampshire, to really make a difference to the mental health and well-being of a diverse range of people across Hart and Rushmoor.
Based in our new Farnborough Well-being Centre, you will be instrumental in building and developing our existing relationships across the Hart & Rushmoor District as well as the area’s Community Transformation Programme. Working closely with our Well-being Manager in Farnborough, you will act as a strategic lead around the Community Transformation Programme in Farnborough developing effective communication with key partners.
You will actively promote the amazing services Andover Mind provides, to raise awareness to local organisations, partners, and the general public, which in turn will encourage those who require support to access our service.
This position also has the responsibility of managing our successful team of Mental Health Community Coordinators and any future roles associated with the Community Transformation Programme.

Job Description Community MH Partnership Manager
PS for Community Mental Health Partnership Manager
This is an exciting opportunity to be part of our successful Well-being teams at Andover Mind who make a difference to individuals with mental health problems.
Our Well-Being Practitioners assess individuals with their mental health and well-being using a strength-based approach to identify and outline with the service user on areas in which they require support. You will support your service users in their recovery through low and high-intensity support structures in conjunction with the recovery star plan and care plans, by providing 1 to 1 and group support. Some of this work may be undertaken within the community as well as within the Well-being centre.
In Addition to the above, this position will also provide a high quality, efficient and effective initial response to all contacts made to the Referrals and Enquiries team, particularly from clients, carers and professionals requiring information and advice. By listening to your service user and talking through their requirements, you will identify the most appropriate pathway that will address their needs. You will also, when identified, implement additional service areas and triage to Well-Being Workers and Dementia Advisers where appropriate.

The role of our Senior Well-being Practitioner (Primary Care) is to support the Well-being Manager with the day to day running of well-being services. To provide recovery focused support to service users in line with the well-being service specification. To manage a caseload of service users, through the delivery of focused one to one support, groups and courses using strength based approach and person centred strategies to meet the individual’s needs of the service user. To record the service users journey using Charity Log and the Recovery Star model. To provide administrative and development support to the Primary Care lead in Andover Mind and line management duties of the Primary Care Well-being practitioners and Peer Support practitioners.
